I have just tried to replicate this using genuine Canon Breech Lock and new FD mounts that are in perfect condition on a scrap FTb. It is impossible to mount the lenses so that the lugs are anyway engaged. With the red dot at exactly the 3 o'clock position the lugs will not engage any slot in the ring. If you mean that you just placed the lens on the camera in the wrong position and one of the lugs has partially stuck in the lens mount, you will just have to wiggle it free. If you can release the shutter, set it on B and look into mirror box from back of camera to check that the lens operating levers are not binding against anything. I would try to manually lift the mirror just in case it is obstructed, before releasing shutter.
If you used brute force and managed to turn the breech ring/lens(nFD), The lens will be seated at an angle to the body because of the location pin. In this case you will have to use one of those rubber strap wrenches that are used for opening screw top jars and try brute force in the opposite direction.
If the lens is a non Canon optic and the location screw/pin is missing it is just possible that you might have got the breech ring to turn. This occurs when the lug cut outs in the breech ring are all machined to the size of the large top mount lug (a cost cutting exercise). If this is the case hold lens and loosen breech ring, then holding breech ring turn lens in opposite direction. This is done to ensure that ring is in the fully opened position. Now holding lens body and ring try and twist the lens back and forth whilst pulling it off body.
