I purchaced a Yashinon Wide Lens (like new in the box) for my Yashica-Mat that went on tight and the adpter ring came loose while taking it off. I had to unscrew it all the way off and use pliers to remove the adapter. Can the adapter be screwed all the way on and locked by what looks like set screws in the adapter or is there some focusing required before locking it down?

Focusing not required. Just screw it back in all the way and tighten those set screws.
