The top of the wind lever doesn't have any holes for a pin wrench to latch onto, how can it be removed (or unscrewed)? Are there any tricks involved, is it a regular right handed thread?

Right hand thread-use something like old inner tube, slip-stop--push down firmly and turn--under you will find a large nut with spanner holes--the rest is basic.
